New Patient Information


Dr. Alarcio is currently taking new patients, although she is always scheduling 2-3 months out. Before your first visit Dr. Alarcio needs (if available) Lab results, Blood Draw results, EEGs, MRIs, Sleep Studies, any mold testing, a Medication list of current and past medications, a brief summary of how the patient’s symptoms began or how the patient has been doing is appreciated.

Dr. Alarcio is not contracted with ANY insurance providers so all office visits will have to be paid out of pocket. Any testing such as Labs, MRIs, sleep studies etc…will be ran through the insurance. The office will not be responsible for any bills sent to you by your insurance for testing performed. The office will provide relevant information to said insurance(s) when they (the insurer) reach out to the office requesting more information.

If insurance(s) require documentation, they will send us an official document via mail or fax requesting documentation which we will provide.

If you are interested in moving forward with seeing Dr. Alarcio, you can call 480-621-8200 and after speaking to the staff they will email you all relevant intake forms. Alternatively, you can download and print the intake forms directly from our website, and when you complete the forms you can send them to info@antichealth.com and we will call to set you up with a placeholder date and the staff will also place you on the cancellation list when we receive said forms. If we do not reach out to you in a timely manner do not hesitate to reach out to us for scheduling.

Cancellation & Rescheduling

If visits are canceled within 24 hours, it will not count against you. Also, since Dr. Alarcio’s office is limited on staff it’s a possibility we may miss your call when you call in to cancel. You can leave a voicemail or send us an email stating you need to cancel the visit and it will not count against you. Please give the office staff 24-48 hours to reach out and assist with rescheduling.

If you no show or cancel more than 4 visits you will be automatically discharged from her Practice.
